Mt. Assisi Place will host its annual flu shot clinic for Residents and employees on Friday, October 23. Representatives from Giant Eagle Pharmacy will be here starting at 1 p.m. to administer flu shots to Residents and employees who wish to receive them.

DETAILS FOR RESIDENTS: Flu shots will be administered to Residents in their rooms. If a Resident wishes to receive a flu shot, families are asked to submit to the Reception Desk by Wednesday, October 14, a completed authorization form and updated copy of the Resident’s insurance information. 

DETAILS FOR EMPLOYEES: Any staff member who wishes to receive a flu shot must submit to the Reception Desk by Wednesday, October 14, a completed registration form and bring a copy of their insurance information with them on October 23. Flu shots for employees will be administered in the large conference room across from Diane Ott's office.
